Families seeking financial assistance for the current season of play must complete the following Funding Declaration form following the completion of the application to the funding source.
Families applying for financial assistance will be responsible to pay JDF Minor Hockey Association the balance of the season fees if the funding source is unable to cover the player’s total registration cost.
